This happens when line endings in the file (sometimes some
parts) are wrong and the binary indexes are slightly out;
but I have not seen it for years and work on Windows
every day.  Can you describe which Acrobat Reader you
use and also show us a script and the output which does
it?

Also, are you using the _rl_accel extension module? Because
we and all our customers always use this, it's possible
we have failed to spot a problem in cases where it is not
used.
